

In 2017, a female child aged 2 years old was admitted to our emergency department following the classic onset of type 1 diabetes. Laboratory evaluation revealed hyperglycemia (glycemia 23.5 mmol/L, HbA1c 64 mmol/mol) with preserved acid-base balance (pH 7.37, HCO3− 24 mmol/L, base excess 0.7). The autoimmune etiology was confirmed (autoantibodies anti insulin, anti-GAD and anti-IA2 were found positive), and her endogenous insulin secretion, measured by fasting C-peptide (0.38 ng/mL), was reduced.
